Your flap reconstruction healed. It works. And still, something is not right: the shape, the symmetry, the donor site scar you notice every day. You may have been told this is simply the result. It is more accurate to say it is the starting point.
Why would a natural tissue reconstruction need revision?
Autologous reconstruction, using your own tissue through procedures such as DIEP, PAP, or LAP flap surgery, offers the most natural look, feel, and longevity of any reconstruction method. But natural does not always mean perfect. Every woman heals differently, not every reconstruction is performed with the same level of precision, and tissues change over time. Issues with symmetry, shape, or donor site healing can emerge months or even years after the initial procedure. Most autologous reconstructions benefit from a secondary revision procedure.
The most common reasons women seek revision:
- Symmetry and positioning. Adjusting the height, projection, and position of the reconstructed breast.
- Shape and contour. Smoothing indentations, refining the natural curve, or addressing volume irregularities through [fat grafting](link: fat grafting page) or liposuction.
- Donor site concerns. Improving scarring, scar placement, or contour at the abdomen or other donor site.
- Functional issues. Correcting tightness, discomfort, or muscle weakness that developed after the initial reconstruction.
Fine is not the finish line
Too often, patients are told they are fine simply because their reconstruction was medically successful, and because, most importantly, they survived breast cancer. We do not accept that standard.
It is rare for breast reconstruction to be fully complete after a single surgery, even in the most experienced hands. Initial breast size and shape, body type, and the need for post-surgical radiation therapy all contribute to the need for revision procedures. These secondary surgeries can feel overwhelming, but they can make a profound difference in your final outcome.

Contour irregularities, indentations, volume asymmetry, and thin coverage. It is one of the most versatile tools in revision, often combined with reshaping and scar revision in a single plan.
There is no one-sized fits all answer to this question. At a minimum, we need to wait until tissues have settled, typically several months after the primary procedure. There is no upper limit however; revisions years later are routine.
