DIEP Flap for Breast Augmentation

The DIEP flap is best known as a breast reconstruction technique, but it can also be used for breast augmentation in the right candidate. The ideal patient is someone who wants the most natural result possible, prefers to avoid implants entirely, and has extra abdominal tissue they would like to address.
How the procedure works
The technique is similar to what we use in breast reconstruction. We harvest the lower abdominal tissue, similar to a tummy tuck, and use it to augment the breast through microsurgical transfer. The result is an entirely natural augmentation. No prosthetic materials. No implant surveillance. No future maintenance. The tissue looks and feels like breast tissue because it is your own.
Who is a good candidate
DIEP flap augmentation is a specialized procedure, and not every patient is a candidate. The best candidates typically share a few characteristics:
- Sufficient abdominal tissue. You have enough lower abdominal tissue to provide the volume you want in your breasts.
- No prior abdominal surgery that compromises the donor site. Procedures such as a previous tummy tuck, extensive hernia repair, or major abdominal surgery may rule out the abdomen as a donor site.
- Willingness to accept a longer recovery. This is a microsurgical operation, with a recovery profile measured in weeks rather than days.
- Goals aligned with a natural result. You are looking for an augmentation that looks and feels like your own tissue, not a specific implant-driven shape or size.
- Good overall health. As with any microsurgical procedure, candidates should be in good general health and able to tolerate the operation and recovery.
If you are unsure whether you are a candidate, we are happy to evaluate you and discuss your options. For patients who are not DIEP candidates, alternative donor sites such as PAP or LAP flaps may be possible.
What to consider
The primary trade-off is a longer recovery compared to implant-based augmentation. This is a microsurgical procedure with the same recovery profile as a DIEP flap reconstruction. For many patients, this is a worthwhile exchange for a result that is permanent, natural, and free from the long-term considerations that come with implants.
