Implant Moves When You Flex? Animation Deformity

You push open a door, lift a bag, or work out, and the implant visibly jumps, flattens, or pulls toward your armpit. Sometimes the skin and nipple move with it. You may never have heard the term for this, but it has one: animation deformity. And it has a fix.

What is animation deformity?

Animation deformity occurs when a breast implant placed behind the pectoral muscle (the submuscular plane) moves unnaturally during muscle contraction. It occurs significantly more often after breast reconstruction than after cosmetic augmentation.

When the pectoralis major activates, during lifting, pushing, or exercising, the implant flattens and shifts upward and outward. In many women this displacement also affects the overlying skin and nipple-areolar complex, which can be visually distressing. Animation deformity can also cause pain and chronic discomfort.

Why does it happen?

Animation deformity has a single root cause: implant placement behind the muscle. As long as the implant remains beneath the pectoralis major, muscle contraction will continue to move it. This is worth stating plainly because it defines the treatment: no exercise, massage, or implant swap within the same plane will fix it.

How is animation deformity corrected?

Treatment moves the implant in front of the muscle (the prepectoral plane) or removes it entirely, along with repairing the pectoralis major back to its natural position on the chest wall. The right combination depends on your anatomy, surgical history, and goals:

  • Implant plane change. The implant is removed, the muscle is repaired to the chest wall, and the implant is replaced in front of the muscle, with additional soft tissue support as needed.
  • Fat grafting. In thinner women or those with thin mastectomy skin flaps, fat grafting before or during plane change improves the aesthetic result, camouflages implant borders, and reduces visible rippling.
  • Biologic or synthetic support. Acellular dermal matrix or specialized synthetic mesh (such as P4HB) reinforces the new pocket and reduces the risk of implant visibility after plane change.
  • Implant type upgrade. Switching to a highly cohesive gel implant can address the rippling and border visibility that are more common in the prepectoral position, often combined with exchange.
  • Conversion to your own tissue. For women who prefer a permanent, implant-free solution, the implant is removed and replaced with natural tissue following muscle repair. For women without sufficient donor tissue, a hybrid approach combining a small flap with an implant improves coverage.

Why does my implant move when I use my chest muscles?

Because it sits behind the pectoralis major, so every contraction of that muscle pushes on the implant. This is animation deformity, and it is a consequence of implant position, not of anything you did.

Does moving the implant in front of the muscle fix it permanently?

Yes. Plane change removes the cause, so the animation does not return. The muscle is also repaired back to the chest wall, which often improves comfort and function.

Won't the implant be more visible in front of the muscle?

It can be, particularly in thin tissue, which is why plane change is frequently combined with fat grafting, soft tissue support, or a more cohesive implant. Managing this tradeoff well is where experience shows.

Can animation deformity be fixed without keeping an implant at all?

Yes. Replacing the implant with your own tissue after muscle repair resolves the animation and eliminates implant-related concerns permanently.
