Heavy, Low, or Uneven Breasts? Breast Reduction and Breast Lift in NYC

Breast reduction and breast lift (mastopexy) belong to the same family of operations. Both reshape the breast and reposition the nipple. A reduction also removes breast tissue to create a smaller, lighter breast; a lift focuses on shape and position while preserving most of the existing volume. A breast reduction incorporates a lift.
At Altris, the plan begins with what you want to change and what your anatomy allows. Every operation is performed by two attending surgeons, Drs. Katie Weichman and Vishal Thanik.
Who breast reduction and breast lift are for
Breast reduction is often considered by women whose breast size causes neck, shoulder, or back discomfort, bra-strap grooves, skin irritation, or difficulty exercising. It can also help women who feel their breasts are out of proportion to their body.
A breast lift addresses changes in shape and position after pregnancy, breastfeeding, weight change, or aging. It may be appropriate when you are comfortable with your breast volume but want to improve sagging, nipple position, or the distribution of that volume.
Differences between the breasts can often be addressed during either operation. We discuss how much improvement is realistic and where some asymmetry may remain.
How we decide between reduction and lift
We measure the breast and chest, assess skin quality and breast tissue, and discuss what bothers you most: weight, shape, position, or a combination. Your exercise, daily activities, and plans for pregnancy or weight change also inform the recommendation.
A reduction removes tissue and reshapes and elevates the breast. The goal is a size that suits your frame and helps address your symptoms. A lift removes excess skin and reshapes the existing tissue, with less emphasis on reducing volume.
We explain the recommended approach, the expected change, and its limitations before you decide.
Can I have a breast lift without implants?
Yes. A breast lift can reshape your existing tissue and improve breast and nipple position without an implant. Implants are not required for a lift.
If you want additional volume or more upper-breast fullness than your own tissue can provide, augmentation may be combined with the lift. We discuss whether that fits your goals and whether the procedures should be performed together or in stages.
What scars should I expect?
The incision pattern depends on how much skin and tissue need to be addressed. Options include an incision around the areola with a vertical extension, the "lollipop" pattern, or an anchor pattern, which adds an incision along the breast fold.
We choose the pattern needed to achieve the intended shape and explain why. A shorter scar is useful only if it allows an appropriate correction.
Scars are permanent. They usually soften and fade over 12 to 18 months, although their appearance varies with skin type, healing, and individual scar response.
What the operation involves and what recovery looks like
Breast reduction and breast lift are generally outpatient procedures. Most patients return to desk work within one to two weeks, depending on the operation and their recovery.
Light exercise may begin around two weeks when healing allows. Running, heavy lifting, and strenuous upper-body exercise require a later, individualized return. We provide specific guidance for your activities.
Swelling and early firmness settle gradually, and the shape continues to develop over several months. Before surgery, we discuss risks including bleeding, infection, delayed healing, changes in nipple sensation, and possible effects on breastfeeding.
Does insurance cover breast reduction or breast lift?
Breast reduction may be covered when breast size causes symptoms and your insurer's medical-necessity requirements are met. Criteria vary and may include symptom documentation, previous treatment, and the amount of tissue expected to be removed. A breast lift performed for cosmetic reasons is generally not covered by insurance.
Altris is an out-of-network practice. For eligible patients with out-of-network benefits, we may be able to arrange care with patient costs comparable to in-network care and handle the insurance process. We review your benefits and explain the applicable arrangement and expected costs before surgery, including surgeon, facility, and anesthesia fees.
What a realistic outcome looks like
Breast reduction can provide lasting relief from symptoms related to breast weight, together with a smaller, lighter, and more proportional breast. Patients often find exercise and everyday activities more comfortable.
A breast lift improves shape and position using your existing tissue, with or without additional volume from an implant. Neither operation stops future changes from aging, pregnancy, or weight fluctuation. Perfect symmetry and a particular bra cup size cannot be guaranteed.
If you have been living with heavy, low, or uneven breasts, schedule a consultation to discuss what can be improved.
FAQ
Prehab is a structured preoperative optimization protocol: a tailored plan to improve strength, nutrition, metabolic health, and other modifiable risk factors in the months leading up to surgery.
The evidence is consistent: patients who arrive at surgery in better baseline condition have fewer complications and recover faster. Prehab is built into every patient's plan from the first consultation.
Posthab is post-surgical rehabilitation. Recovery does not happen by accident, so we continue the work that began with prehab.
Our posthab protocol supports a smoother, more complete recovery through guided return to activity, nutritional support, and ongoing assessment of how the body is responding.
Treatment depends on the cause and your goals. Where possible, we adjust the less favoured side to match the preferred outcome.
- Implant exchange or revision — removing and replacing the implant to improve size, shape, or position
- Reshaping after autologous reconstruction — refining the footprint, projection, and contour of a flap reconstruction
- Fat grafting — transferring your own fat to areas where volume or contour need improvement
- Mastopexy or breast reduction — lifting or reshaping the natural breast to match the reconstructed side
- Nipple reconstruction and tattooing — particularly useful in unilateral reconstruction

our philosophy
After years in clinical practice, we have watched patients complete treatment, receive a clean bill of health, and still feel like strangers in their own bodies. Fragmented care, time-pressured appointments, and a healthcare system focused on disease management rather than whole-person recovery leaves too many people stranded between surviving and truly living.
