How long does breast reconstruction take? Recovery timelines by procedure

Recovery from breast reconstruction varies significantly depending on the procedure. The timelines below reflect our protocols at Altris, including our Enhanced Recovery After Surgery (ERAS) program for autologous reconstruction and our in-house prehab and posthab protocols, which we use for all patients to optimize both preparation and recovery.
It is worth noting that breast reconstruction is rarely a single surgery. Most patients undergo a primary procedure followed by one or more refinement procedures over time. Each stage has its own recovery timeline, and we plan these carefully with each patient from the outset.
DIEP, PAP, and LAP flap reconstruction
Autologous flap reconstruction is the most involved of the procedures we perform, and recovery reflects that. That said, most patients are surprised by how manageable the process is with proper preparation.
- Hospital stay: 2 to 3 nights
- Drain removal: 7 to 14 days (LAP flap: 3 to 4 weeks)
- Return to light activity / desk work: 2 weeks
- Return to driving: 3 weeks
- Return to full activity and exercise: 4 to 6 weeks
- Final result visible / revision considered: 3 months
The LAP flap has a slightly longer drain and overall recovery timeline than the DIEP or PAP due to its technical complexity and the use of vascular grafts.
All autologous reconstruction patients at Altris are managed under our ERAS protocol, which is designed to reduce pain, minimise complications, and accelerate recovery through optimised perioperative care.
Implant exchange
Implant exchange is an outpatient procedure with a straightforward recovery for most patients.
- Hospital stay: Outpatient (home same day)
- Drain removal: Not always required; if used, 7 to 10 days
- Return to light activity / desk work: 2 weeks
- Return to driving: 2 weeks
- Return to full activity and exercise: 4 weeks
- Final result visible / revision considered: 3 months
Secondary revision procedures
Recovery from revision surgery depends on the complexity of the procedure. We broadly categorise revisions as minor or major, and timelines differ accordingly.

Fat grafting
Fat grafting is one of our most straightforward procedures in terms of recovery, and is frequently performed alongside other revision procedures.
- Hospital stay: Outpatient (home same day)
- Drain removal: No drain required
- Return to light activity / desk work: 1 week
- Return to driving: 1 week
- Return to full activity and exercise: 3 weeks
- Final result visible / revision considered: 6 to 8 weeks
Note: approximately 60% of transferred fat remains viable at 6 to 8 weeks. Multiple sessions are often needed to achieve the optimal result.
What is prehab and posthab, and why does it matter?
At Altris, all patients participate in our in-house prehabilitation (prehab) and post-surgical rehabilitation (posthab) protocols. Prehab involves optimising your strength, nutrition, metabolic health, and overall fitness before surgery. Posthab supports your recovery and return to full function afterward.
We have found that patients who are well-prepared before surgery recover faster, experience fewer complications, and achieve better long-term results. These protocols are built into every patient's plan from the first consultation.
A note on the overall reconstruction journey
The timelines above reflect individual procedures. Breast reconstruction as a whole, from initial surgery through final refinement, typically spans multiple stages over one to two years, sometimes longer depending on treatment history, the need for radiation, and individual healing. We discuss the full anticipated journey with every patient during consultation so there are no surprises.
